  • What is it?

The leave donation policy at UNF was designed to assist employees who are experiencing medical conditions resulting in a leave without pay status. The employee must have exhausted all of their accrued sick and annual leave before applying for the leave donation pool.

If you are interested in donating to the pool, please complete the leave donation form and return it to the Office of Human Resources.

If you are interested in requesting leave, please complete the leave donation application and have your physician complete the certification of health care provider form. Return both to HR.

  • Who is eligible to participate?
An employee meeting the requirements of the leave donation policy may receive donated leave if the employee or his or her eligible family member is experiencing a serious medical condition covered under the Family and Medical Leave Act of 1993. The employee must have exhausted all their accrued sick and annual leave before applying for the leave donation pool.
