School of Computing - Forms

The School of Computing forms have gone PAPERLESS.

• Please fill out your form electronically (no handwriting unless necessary).
• If you are submitting a form that has multiple pages, you must consolidate all the pages of the form into one PDF file.
• If a form requires instructor's signature (e.g., Registration Waiver Request form or a Directed Independent Study form), be sure to complete your part and sign the form before emailing the instructor to collect their signature. Likewise, follow the same procedure for the internship form to collect the supervisor’s signature.
• If your form does not require an signatures it can be filled out online.

 Academic Plan of Study

• Form to assist students in designing a plan of study to fulfill their major requirements.  
• Send completed form by email to computing@unf.edu

Academic Plan of Action

• Form to intervene when a student receives a probation notice (warning) due to failing to maintain good academic standing or for violating the Satisfactory Progress Policy.  
• This form is submitted online using student's MyWings credentials.

Admission Referral Form

• Form to  Aid in determining whether to accept or deny student's admissions referral request.
• This form is submitted online using student's MyWings credentials

Directed Independent Study

• Request permission to take a CIS4900 or CIS6900 Directed Independent Study course.
• Make sure to collect the instructor's signature before uploading the form.
• This form is submitted online using the student's MyWings credentials.

Computing CO-OP & Internship Agreement

• Request to register for CIS3949 or CIS5949 Experiential Studies in Computing.  Read responsibilities carefully.  
• Make sure you collect the internship supervisor's signature before uploading the form.  This form is submitted online using student's MyWings credentials.
• Please consult your advisor with any questions.

Registration Override Request

• Request to take a course without the appropriate prerequisites, or with time-overlap with another course.
• Make sure you collect the instructor's signature before uploading the form.
• This form is submitted online using student's MyWings credentials.

Return from Suspension Request

• Request to return from suspension and intent to major in a computing major upon returning. *Other desired majors will need to look for the appropriate unit's from on the UNF Academic Advising site.
• This form is submitted online using the student's MyWings credentials.

Oral Communications Skills Assessment

• Required to fulfill the Oral Communication requirement of all Computing majors.  While most students complete this requirement in CIS3253, Legal and Ethical Issues in Computing, students may obtain the permission of an instructor in any upper-level (3000-4000) computing course to fulfill this requirement.
• Make sure to collect the instructor's signature before uploading the form.
• This form is submitted online using student's MyWings credentials

Waiver of School Policy

• Request an exception to School Policy due to extenuating circumstances.
• You can attach all supporting documents for your request as one pdf file.
• This form is submitted online using student's MyWings credentials.
