In-Clinic Visits  

Please schedule an appointment online on your student Patient Portal or by calling 904-620-2900 before completing your forms.

Please complete your forms at least 24 hours in advance if possible. If scheduling a same-day appointment, please complete your forms as soon as possible.  

Please do not fill out these forms before you have scheduled an appointment. These forms must be submitted at least 1 hour prior to your appointment or your appointment may need to be rescheduled.

Telehealth Visits

You will receive a text and/or email with the appointment link no later than 1 hour prior to your appointment.  You may check-in to the virtual appointment as early as 15 minutes before your scheduled appointment time.  

We use a telehealth service called Pathways.  It works best with a fast internet connection while using a PC/Mac with a webcam.  You may also use a smartphone.  Your device must have a video camera and microphone.   

We have noticed sound difficulties when Apple AirPods are used.  We have also noticed that a phone's battery may drain quickly while on a telehealth appointment. 

Preparing for your telehealth appointment.   

•  Choose a location that is quiet and will maintain your privacy.  Do not call while you are driving. You will be asked to reschedule your appointment. 
•  Write down your symptoms and how long each one has been present.  
•  Put medical devices close at hand. If you have a thermometer, blood pressure monitor, or device to check your heart rate (such as a smart watch or pulse oximeter) we may request a current reading.  
•  Make a list of any questions you have for the clinician or nurse.
