In-Clinic Visits
Please schedule an appointment online through your student Patient Portal or by calling 904-620-2900 before completing your forms.
Please complete your online forms at least 24 hours in advance if possible. If scheduling a same-day appointment, please complete your forms as soon as possible.
Most appointments are only 20 minutes in length. If you are more than 10 minutes late after your appointment start time, you will be charged $15.00 and will need to reschedule.
You will be charged $15.00 for a No Show, Late Cancellation, Same Day Late Cancellation, or Late Arrival (see details in policy below). You must arrive 10 minutes before your appointment start time.
Telehealth Visits
You will receive a secure message through your student Patient Portal with the Zoom appointment link no later than 1 hour prior to your appointment. You may check-in to the virtual appointment as early as 15 minutes before your scheduled appointment time.
We use Zoom for telehealth. It works best with a fast internet connection while using a PC/Mac with a webcam. You may also use a smartphone. Your device must have a video camera and microphone.
We have noticed sound difficulties when Apple AirPods are used. We have also noticed that a phone's battery may drain quickly while on a telehealth appointment.
Preparing for your telehealth appointment.
- Choose a location that is quiet and will maintain your privacy. Do not call while you are driving. You will be asked to reschedule your appointment.
- Write down your symptoms and how long each one has been present.
- Put medical devices close at hand. If you have a thermometer, blood pressure monitor, or device to check your heart rate (such as a smart watch or pulse oximeter), we may request a current reading.
- Make a list of any questions you have for the clinician or nurse.
No-Show, Late Cancellation, Same Day Late Cancellation, and Late Arrival Policy
Purpose:
To ensure efficient scheduling and availability of appointments for all patients, this policy outlines the procedures and fees associated with missed appointments, late cancellations, same day cancellations, and late arrivals.
Description:
“No Show” shall mean any patient who fails to arrive for a scheduled appointment.
“Late Cancellation” shall mean any patient who cancels an appointment less than 24 hours before the scheduled appointment.
“Same Day Late Cancellation” shall mean any patient who cancels a same day appointment either more than 30-minutes after scheduling or after the scheduled appointment time, whichever occurs first.
“Late Arrival” shall mean any patient who checks in for an appointment more than 10 minutes after the scheduled appointment time, resulting in a missed appointment.
Policy:
It is the policy of Student Health Services (SHS) to monitor and manage appointment no-shows, late cancellations, same day cancellations, and late arrivals. The SHS’s goal is to provide excellent care to each patient in a timely manner. If it is necessary to cancel an appointment, patients are required to call, leave a message, or cancel through the patient portal at least 24 hours before their appointment time. Patients who schedule a same day appointment must cancel within 30 minutes of making the appointment or before the scheduled appointment time, whichever occurs first. Notification allows the clinic to better utilize appointments for other patients in need of prompt medical care.
Procedure:
Patients who fail to communicate an appointment cancellation and fail to show for a scheduled appointment will be coded as a No Show and charged the No Show/Late fee.
Patients who fail to communicate an appointment cancellation less than 24 hours prior to the scheduled appointment time will be coded as a Late Cancellation and charged the No Show/Late fee.
Patients with a same day appointment who fail to communicate an appointment cancellation either within 30 minutes of making the appointment or before the scheduled appointment time, which ever occurs first, will be coded as a Same Day Late Cancellation and charged the No Show/Late fee.
Patients who fail to check-in within 10 minutes of a scheduled appointment time, resulting in a missed appointment, will be coded as a Late Arrival and charged a No Show/Late fee.
The No-Show/Late fee is:
- General Medical, Nurse Visit, and Lab Appointments: $15 per occurrence
- Gynecological and Specialty Appointments: $15 per occurrence
The No Show/Late fee will be added to the patient’s Student Health Services bill. Payment is due within 30 days of the charge being posted.
Exceptions for emergencies or extenuating circumstances may be reviewed on a case-by-case basis.
Enforcement:
By scheduling an appointment, patients agree to this policy. Questions or appeals regarding fees may be directed to the Director, Student Health Services, by emailing StudentHealth@unf.edu.
