Obtain the Form DS-2019

Items needed for the Form DS-2019

The Form DS-2019 is generated by the Student Exchange and Visitor Information System (SEVIS) and is associated with a particular SEVIS ID. This form certifies that the exchange visitor has been screened and selected for participation in the exchange visitor program, identifies the exchange visitor and their designated sponsor, provides a brief description of the program, and permits the exchange visitor to apply for the J-1 visa to enter the United States.

To have your Form DS-2019 issued, please submit the items listed below via email to intlctr@unf.edu. When sending the email please write your "full name/school/DS-2019" in the subject line. Example: Jose Perez/University of Malaga/DS-2019.

  1. Certification of Financial Responsibility

    Prior to issuing your Form DS-2019, we must confirm availability of funds for the entire length of your exchange program. We do this via the Affidavit of Financial Support form. You must submit an Affidavit of Financial Support with the amount listed on the Affidavit of Financial Support webpage for the semester(s) in which you are attending. For example, if your program at UNF is more than one semester, you will need to submit an Affidavit for the whole length of study by doubling the semester price.  A list of acceptable financial documents can be found in the Forms section.

    When filling out the affidavit form, indicate the start and end date of the semester(s) you are planning to attend UNF. Please find the semester start/end dates at the UNF academic calendar.

    Please note that the affidavit figures are estimates only, not actual costs. Because housing, and food costs tend to increase every year, it is recommended that you budget for an additional 5% to 10% over the estimated costs. If you plan to enroll for the summer term along with the fall and spring terms, you should budget for at least an additional $8,000 to $13,000 depending upon the number of credits you register for and your academic level.

    If you are bringing a spouse and/or children to the U.S. on a J-2 dependent visa, you must show an additional $1,750 per dependent per semester to cover their estimated living expenses.

  2. Copy of Passport

    A scanned copy of your (and your dependents') passport bio page is necessary to ensure that the information indicated on your (and any dependents') Form DS-2019(s) matches the information of your passport(s).

Receipt of Your Form DS-2019

Once you have submitted all the required information and documents indicated above, we will mail your Form DS-2019 to your school.

Review and Attestation of Your Form DS-2019

Although our office reviews your Form DS-2019 for accuracy, you also need to review it, after you have received it, to verify that the biographical information on your (and any dependents') Form DS-2019(s) matches the information of your passport(s).

If the information does not match, email the International Center as soon as possible with an explanation of the correction needed. We will send you a corrected Form DS-2019.

Once you have confirmed the accuracy of your Form DS-2019, sign and date it at the bottom of the first page.

After receiving the DS-2019, students will need to pay the SEVIS fee (information regarding the fee will be included with the DS-2019) and contact the appropriate U.S. Embassy/Consulate to verify the current application procedures and the appropriate method to submit the necessary materials to schedule the interview and obtain the visa.
