  • Opt Out
    • This form is used when you do not want to use you benefit for a single term.
  • Parent Letter Request 
    • A Parent Letter is required from UNF IF you are attending a different school to fulfill your degree requirement at UNF.
    • Failure to submit this form before enrolling at another institution may result in delayed payment, denial of VA funding for the course, or removal from the class due to nonpayment.
