University-sponsored Camps

(a) The State of Florida provides general liability insurance for UNF employees, agents and volunteers,
(b) Proof of accident/health insurance must be included as an attachment to the application package for University-sponsored camps. This insurance may be purchased via the Department of Management Services (DMS).

Non-sponsored Camps (Affiliate and Third-Party)

(a) Proof of liability insurance, in the form of a “certificate”, must be included as an attachment to the application package for all Non-sponsored camps (Affiliate and Third-Party).  This certificate of evidence of liability insurance must name the University of North Florida a Public Body Corporate, the UNF Board of Trustees, the State of Florida and their officers, agents and employees as additional insureds by written endorsement. A minimum of $1,000,000 coverage per occurrence and $2 million in the aggregate is required.

 (b) Proof of accident/health insurance must be included as an attachment to the application package.

Reference the specific UNF facility rental agreement for liability insurance coverage requirements.  Requests for exceptions to this minimum may be submitted to the Director of EH&S, who will note on the Camp Application if the request is granted. Final approval for any exception rests with the Vice President for Administration and Finance.

Agreements & Releases

CAMPS Facilities Use Agreement Facilities use agreement intended for all camps and other similar programs using general university facilities.

CAMPS Facilities Use Agreement - Arena, Stadium, Recreation - Facilities Use Agreement intended for all camps and other similar programs using the Arena, Stadium and Recreation facilities.

Third Party Release - Non-UNF entity that operates a camp/program on UNF's campus.  This release is to be signed by the non-UNF entity, not by camp/program participants and sent to the UNF host, to be retained on file for a period of 5 years.
