In order to obtain the fingerprinting/level 2 screening necessary for summer camp staff, please submit the following documents to HR in order to begin the screening process for each potential staff member.  Each staff member must submit both the Care Provider Background Screening Clearinghouse Request Form and Privacy Policy Acknowledgement Form

Staff List/ Background Screening

This list must be included in the camp/program application package and submitted to the Office of Youth Programs at at least six (6) weeks prior to the start of a camp/program. Camp Employee Background Checks 

Outside Activity Report

This form is accessible via the myWings portal. You must submit this form if you are a 9 or 12 month faculty member, full-time A&P or USPS staff member and if: you propose to engage in any compensated or professional outside activity as described on the form and/or you propose to engage in any outside activity, compensated or uncompensated, for which you should reasonably conclude: may create or reasonably appear to create a conflict of interest, OR may otherwise interfere or reasonably appear to interfere with the full performance of your professional responsibilities or other institutional obligations. 

Affidavit of Good Moral Character

Required for all adult participants in camps/programs. This document is to be kept by the camp director.
