Videography Request Form

This Video Request Form must be submitted no later than forty-eight (48) hours prior to the requested event. Submission of this request is not a confirmation of reservation. You will be contacted about your request within two (2) business days. You must also fill out the Recording Release and License form [pdf] and follow the instructions indicated on the Recording Release Form. Speakers affiliated with the University do not need to fill out the Recording Release Form.

Agreement
	I understand that I must make my request at least forty-eight (48) hours in advance in order for Coggin College Technical Support Office to prepare for my event. I also understand that Coggin College Technical Support does not provide minidv tape(s) for recording events and that I must provide minidv tape(s) to be used to video tape my requested event.
	I understand that no distribution of the video will be possible unless the Recording Release and License form is signed and returned to the Coggin College Technical Support Office.
	I understand that submission of this form does not automatically confirm that the requested event will be recorded and/or distributed.
	I affirm that this event is not a performance and understand that Coggin College Technical Support Office cannot record performances.

You must also reserve the equipment through the Coggin College Scheduler in the Tech Office. Minidv tape(s) must be provided in order for the event to be recorded. Coggin College Technical Support does not provide minidv tape(s) for the taping of events.
