Use this page to identify the correct UNF office for reporting incidents, concerns, or concerning behavior. If you are unsure where to report, contact the UNF Hotline, which accepts reports on any topic and allows anonymous submissions.

UNF Hotline
Hotline Services - Office of Internal Auditing
Use the UNF Hotline to report suspected waste, fraud, fiscal mismanagement, research noncompliance, or other similar concerns. The University's third-party reporting service provides a safe, secure, and confidential way to submit reports, including anonymously.
Please note: This is not an emergency hotline and is not monitored 24 hours a day.
Reports are reviewed by the Office of Internal Auditing to determine whether they qualify for whistleblower protection or should be referred to the appropriate University office for follow-up. Reports are handled in accordance with UNF Regulation 1.0110R Fraud Prevention and Detection and Florida Statute 112.3187 (known as the whistleblower Act) are applicable.
Sexual Misconduct, Harassment, and Discrimination
Office of Title IX and Civil Rights (OTCR)
For reporting sexual misconduct, sexual harassment, discrimination or other concerns within the jurisdiction of the Office of Title IX and Civil Rights.
Campus Security Authority: Crime and Incident Report Form
For Campus Security Authorities (CSAs) to report crimes disclosed to them that may be reportable under the Clery Act.
Silent Witness
University Police Department (UPD)
For reporting crime or information concerning a crime that occurred either on- or off-campus.
Student Complaints and Concerns
For reporting general complaints and concerns regarding the University. Not to be used for concerning student behavior, student conduct violations, or grade appeals.
Supporting Our Students (SOS) Referral
Dean of Students Office – Care Services Team
For referring a student who is exhibiting significant changes in behavior or appearance, signs of distress, housing or food insecurity or other concerns about a student's well-being.
Student Conduct Reporting Form
Dean of Students Office – Student Conduct
For incidents involving behavioral Student Code of Conduct violations, including, but not limited to, hazing, alcohol or illegal drugs, disruptive behavior, or violations of UNF regulations and policies.
Classroom Accommodation Concerns
Student Accessibility Center (SAC)
For concerns regarding accommodations requested and documented through SAC.
Academic Misconduct Reporting Form
Dean of Students Office – Student Conduct
For incidents involving academic misconduct Student Code of Conduct violations, including, but not limited to, plagiarism, misuse of materials, and unauthorized collaboration.
