Appointment Information
Please schedule an appointment to see a medical provider by calling in advance. Please call the front desk at 904-620-2900 for all appointments.
Patients
with acute urgent care concerns or injuries may walk in for a clinic visit. An
assessment will be made and measures taken to quickly provide the appropriate
level of medical care.
Women's
Health Annual exams are scheduled approximately 2 weeks in advance.
Travel
Health appointments are scheduled during regular clinic hours. An appointment
with the travel health nurse is required prior to receiving medical
treatment/vaccinations.

Appointment Policy
All
patients are requested to arrive at least 10 minutes prior to the scheduled
appointment time to allow for form completion and self-check-in at the
electronic kiosk. If you do not arrive on time for your appointment, your appointment
may be canceled, and you will need to reschedule.
If you are
unable to keep your appointment, please call and cancel. There may be a $10 fee imposed for missed appointments,
which have not been canceled or rescheduled.
UNF Student
Health Services (SHS) clinical areas qualify as “hazardous locations” where
children are not permitted. Children are permitted in the SHS lobby/waiting
area if they are supervised at all times by an adult. If a patient arrives with
a child, he or she will be asked to reschedule his or her appointment. The
patient may then return without the child or with an adult who can supervise
the child in the waiting area. The patient may have to reschedule and may be
charged a “missed appointment” $10 fee.
Reminder: During summer session, there is a $30.00 fee for all appointments or visits when not enrolled in classes.
UNF Student Health Services licensed professional healthcare employees are subject to immunity within the scope of their employment with UNF (see Florida Statute 768.28).
Telehealth Appointments

What to do once
you've scheduled a telehealth appointment with UNF Student Health
Services:
Please complete
the following: Student Health Services Patient Portal Forms. These
forms must be submitted at least 1 hour prior to your
appointment or your appointment may need to be rescheduled.
You will receive
a text and/or email with the appointment link no later than 1 hour prior to
your appointment. You may check-in to the virtual appointment as early as
15 minutes before your scheduled appointment time.
We use a telehealth
service called Pathways. It works best with a fast internet
connection while using a PC/Mac with a webcam. You may also use a
smartphone. Your device must have a video camera and
microphone.
We have noticed
sound difficulties when Apple AirPods are used. We have also noticed that
a phone's battery may drain quickly while on a telehealth appointment.
Preparing for
your telehealth appointment.
- Choose
a location that is quiet and will maintain your privacy. Do not
call while you are driving. You will be asked to reschedule your
appointment.
- Write
down your symptoms and how long each one has been present.
- Put
medical devices close at hand. If you have a thermometer, blood pressure
monitor, or device to check your heart rate (such as a smart watch or
pulse oximeter) we may request a current reading.
- Make a
list of any questions you have for the clinician or nurse.
