Coronavirus Administrative Leave: Self-Quarantine

Faculty and staff who have traveled to or have been in close contact (e.g., being within 6 feet for longer than 1-2 minutes) with someone recently returning from countries with CDC Level 2 and Level 3 rating are required to quarantine at home and avoid contact with others during a period of 14 days, even if no symptoms of illness are present. Those traveling from any country with over 100 cases of COVID-19  or returning from cruise ship travel are strongly recommended to quarantine at home as well. Employees who are (or have been in close contact with someone) exhibiting symptoms consistent with the published symptoms of COVID-19 or have been recently tested for COVID-19 should self-quarantine and monitor symptoms during the 14-day period. Symptoms can be monitored and documented using the Symptom Monitoring Form.   See below for table detailing the type of self-quarantine required based on the severity of risk. 




If you must self-quarantine:

  • Submit the online Self-Reporting Form
  • The 14-day period begins at the time of HR designation of self-quarantine.
  • Complete the Symptom Monitoring Form to track your symptoms. The Symptom Monitoring Form will be required to be released to return to work, for those that have observed the self-isolation/quarantine period as a precaution, but were not tested for COVID-19.
  • Do not enter the UNF campus for class, work, or any other activity.
  • Employees are expected to work remotely if applicable for job functions.
  • Notify HR immediately if diagnosed with COVID-19.
  • Medical release is required before returning to work for those that have been tested for COVID-19.


Employees who believe they meet the above criteria and/or are experiencing symptoms of coronavirus infection should complete the online Self-Reporting Form, contact their supervisor and Human Resources at (904) 620-2903, Human Resources will confirm next steps and ensure proper designation of leave. Supporting travel (or other relevant) documentation evidencing potential of exposure will be requested prior to leave approval.

Remain under quarantine authority; no public activities; daily active monitoring; controlled travel.
