Workers' Compensation

 

 

As of January 1, 2014, AmeriSys, has assumed responsibility for handling workers' compensation claims for the University of North Florida. All work-related injuries and illnesses must be reported to AmeriSys regardless of the need for medical treatment. Employees contacting AmeriSys will speak directly with a triage nurse employed by the Medical Case Management Provider (AmeriSys) who will complete the First Report of Injury or Illness Form and direct the employee to the nearest medical facility to seek treatment, if necessary. If the injured employee is unavailable to contact Amerisys, the employee's supervisor is responsible for contacting Amerisys on behalf of the injured employee. Please note, UNF's location code is 0262. AmeriSys is available 24/7/365.

  •  AmeriSys will provide copies of all doctors notes directly to the University Workers' Compensation Administrator.
  • The University Workers' Compensation Administrator will work as the liaison between the Nurse Case Manager, State Adjuster, and the University to ensure updates on the injured workers' medical treatment and indemnity benefits are accurate and communicated.
  • The Workers' Compensation Administrator will work with the department and injured employee if the injury prevents the employee from returning to work or causes specific functional limitations.
  • The injured employee should forward any receipts and bills received directly to the Workers' Compensation Administrator.
Important to Note: Please visit UNF's Environmental Health and Safety website to complete an accident investigation form following any injury.

 All employees should familiarize themselves with the below steps to take in the event of a work-related injury:

Injury Requiring Immediate, Emergency Medical Treatment

If an injury is a medical emergency, have the injured employee transported to the nearest medical facility or call campus police for assistance. The injured employee's supervisor is responsible for calling Amerisys if the injured employee is unable to. To contact Amerisys, call (800) 455-2079 immediately following the arrangement of emergency medical services.

 

Injury Requiring Non-emergency Medical Treatment

If an injury is not a medical emergency but does require some degree of medical treatment, the injured employee must immediately notify their supervisor. After the injured employee notifies their supervisor, they are required contact AmeriSys at (800) 455-2079 to submit a claim. AmeriSys will direct the injured employee to the appropriate medical facility to receive treatment.

Injury Requiring No Medical Treatment

If an injury is not a medical emergency and does not require medical treatment, the injured employee remains obligated to notify their supervisor. After the injured employee notifies their supervisor, they are required to contact AmeriSys at (800) 455-2079 to submit a claim.

 

All injuries must be reported to AmeriSys, regardless of the need for medical treatment.
