The Outside Activity form has recently undergone significant updates and is renamed to Conflict of Interest Disclosure. Effective August 2021, employees are required to submit a conflict of interest disclosure annually (at the beginning of each fiscal year or as soon thereafter as possible) to disclose reportable matters or to confirm that they do/do not have relatives employed at UNF and/or participate in outside activities. Employees are also required to submit additional conflict of interest disclosures as soon as possible when proposing to engage in a new reportable manner or there is a significant change in the reportable manner previously disclosed (nature, funding, etc.). Upon approval or denial, the employee will be notified of the outcome.
