UNF is asking all members of the campus community who have recently traveled domestically or internationally or have been in close contact with someone who recently traveled to self-isolate at home and avoid contact with others during a period of 14 days, even if you have no symptoms of illness. UNF also requests that all members of the campus community who have recently traveled to any hotspots within the United States (for example, New York) or taken a cruise to self-isolate at home for 14 days. Please complete the appropriate self-reporting form below. The first one listed is for students; the other is for faculty/staff. When in doubt about whether to complete a self-reporting form, please complete the form as we would prefer more rather than less information to help keep the community safe. Please monitor temperature and symptoms 2 times daily using the 14-day self-isolation observation form.

 

Students who need to self-quarantine should reach out to their professors to arrange ways to complete their academic obligations remotely as well as call UNF Student Health Services at (904) 620-2900. We are also asking all students who self-quarantine to fill out UNF's online Student Self-Reporting Form.

Faculty and staff who need to self-quarantine should contact UNF Human Resources at (904) 620-2903 to ensure proper designation of leave and receive important information about work arrangements. We are also asking all employees who self-quarantine to fill out UNF’s online Faculty and Staff Self-Reporting Form.
