Equal Opportunity and Inclusion
Report an incident involving an alleged violation of UNF's Non-Discrimination, Equal Opportunity and Diversity Regulation (including discrimination or discriminatory harassment on the basis of age, color, disability, marital status, national origin, race, pregnancy, religion, sex, sexual orientation, or veteran status) by any member of the UNF community.
Commission on Diversity & Inclusion
Anyone who witnesses or views evidence of an act of intolerance is encouraged to report the incident. A person need not be a direct target of intolerance to report it. The purpose of this form is for CODI to identify the frequency and types of intolerance incidents that occur on campus. This form does not replace reporting the incident to the University Police Department, EOI Office, Victim Advocate, Ethics Hot line, Counseling Center, or other university offices. If the incident is a criminal violation, it should be reported to the University Police Department. If the incident is discrimination, it should be reported to the Office of Equal Opportunity & Inclusion.
Dean of Students
Submit a referral if you have concerns about a student that encompass any of the following: mental health concerns including depression or anxiety; bizarre behavior changes such as inappropriate display of anger or extreme sadness; suicide ideation or attempt; significant changes in appearance, hygiene, or behavior; and housing/food insecurity. Office of Student Accountability and Resolution (OSAR)
Report a violation of the Student Code of Conduct, including but not limited to: hazing, harassment, alcohol or illegal drugs, weapons, disruptive behavior, or violations of other UNF regulations and policies. Find the Student Code of Conduct below.
OSAR and Human Resources
Report an individual you believe is not following the Health and Safety Measures established in the Coronavirus Safety Policy.
Report fraud, compliance and ethics concerns with Lighthouse, a third-party hotline provider, to ensure the safe, secure and anonymous reporting. Find additional information about the Ethics Hotline below.
University Police Department (UPD)
Report a crime or information concerning a crime that occurred either on or off campus.
Student Accessibility Services (SAS)
Report any accommodation concerns.
Academic Affairs (Provost Office)
To report a student incident of academic misconduct by a professor, please complete the Academic Misconduct Reporting Form. Faculty are required to send the student involved this form after appropriate signatures are required and include the link to the Academic Appeals process due to Misconduct.
Equal Opportunity and Inclusion (EOI)
Report an incident involving an alleged violation of UNF’s Sexual Misconduct and Title IX Sexual Harassment Regulation (including gender-based assault, harassment, exploitation, dating and domestic violence, stalking, as well as discrimination based on sex, sexual orientation, gender identity, and gender expression, and related retaliation) by any member of the UNF community including Responsible Employees.
