Anonymous and Confidential Reporting

Anonymous Reporting

You may choose to make an anonymous report if you want someone in the University to be aware of your experience, but do not want to be involved in the Conduct Process and/or investigation. Please note by submitting a report anonymously, UNF may be limited in its ability to investigate or respond to the incident if it does not have sufficient information from which to follow up on. By reporting anonymously, either as a witness or a victim, you will not be identified as the individual who submitted the report.

 

When submitting a referral please include the following information to the best of your ability:

  • Specific Date(s), Time(s) and Timeline
  • Names of Location(s)
  • Name(s) of those involved and at what capacity
  • Detailed narrative of events
  • Photos, screen shots, videos, etc.

Confidential Report

You may choose to submit a confidential report, meaning any of your identifying information will not be disclosed and/or shared: Name, Phone number, etc.

 

When submitting a referral please include the following information to the best of your ability:

  • Specific Date(s), Time(s) and Timeline
  • Names of Location(s)
  • Name(s) of those involved and at what capacity
  • Detailed narrative of events
  • Photos, screen shots, videos, etc.

UPD Silent Witness

If you see a crime occur or you have information concerning a crime that occurred either on or off campus, you may submit a Silent Witness form. This report may also be submitted anonymously, or if you decide to provide your name for additional follow-up, will remain confidential.
