Recombinant human growth hormone (rhGH): effects on metabolic profile, body composition and skeletal muscle strength and function in prepubertal boys with and without GH deficiency. The growth promoting effects of rhGH are already well established in a multiplicity of conditions including GH deficiency, Noonan syndrome, Prader-Willi syndrome, Turner's syndrome, chronic renal insufficiency, children born SGA and in idiopathic short stature. However, there are very limited data available about the effects of rhGH in areas other than growth promotion in children. A few studies, however, show improved lean body mass accrual leading to more favorable body composition and increase in skeletal muscle strength and agility in a subgroup of very young patients with Prader-Willi syndrome. Our study is specifically designed to measure the effects of rhGH, in addition to growth promotion, in children with both growth hormone deficiency (GHD) and idiopathic short stature (ISS), as compared to the natural changes related to normal growth in age-matched healthy controls not on rhGH. We would limit this study to prepubertal boys to eliminate the confounding effects of rapidly rising sex steroids and to have a realistic sample size to study these effects within the time limitations of the study, as we see far more boys than girls with growth disorders. Specific aims: We wish to study if rhGH administration for 6-12 months in prepubertal boys with profound short stature due to either GHD or ISS positively impact: 1) lean body mass accrual; 2) bone mineral density; 3) skeletal muscle strength; 4) muscle agility and; 5) resting energy expenditure. $100,000

(2015-17) - Nemours Children’s Clinic Internal Research Grant - Nemours Children’s Hospital, Jacksonville, FL. Exercise Snacks and Glutamine to Improve Glucose Control in Adolescents with Type 1 Diabetes. In this proposal we will study the feasibility and efficacy of the use of exercise and dietary supplementation with a non-essential amino acid – glutamine – a component of most protein supplements, on the regulation of plasma glucose homeostasis in a clinical setting of children with type 1 diabetes (T1D). Dr. Churilla will be an advisor on all aspects of the exercise portion of these studies. He will advise, teach and also model the exercise by providing a short video on how the best way to perform these activities to be doable and feasible for the participants at their homes. Dr. Churilla will be involved in the analytical portion of the studies as well. $20,000. Roles: PI Darmaun, D. and Mauras, N. Exercise Physiologist / Consultant / Churilla, JR.
