Reporting Concerns

 

UNF is committed to providing a safe, secure, and ethical work place. Effective ethics is a team effort involving the participation and support of every employee. 

As a member of the UNF Osprey Community, you have a responsibility to voice fraud, compliance and ethics concerns.

To help you voice concerns, UNF has engaged Lighthouse Services—a third party hosted hotline provider—to enable the safe, secure, and anonymous reporting of activities which may involve 

misconduct, fraud, abuse, and other violations of UNF policies. 

 

The UNF Triage Committee reviews hotline cases.  The Triage Committee is comprised of Internal Audit, General Counsel, Human Resources, Equal Opportunity and Diversity, and University Compliance.

This hotline is NOT a substitute for routine communications between employees and supervisors. 

Likewise, it does not replace communications with Human Resources staff about benefits or other job issues.

UNF management does not condone and prohibits retaliatory action against anyone who reports or inquires, in good faith, about potential breaches of UNF policies.

What should be reported?

  

Financial Matters

Misuse, mismanagement or misappropriation of funds, supplies or any other asset (including equipment). Accounting irregularities, fraud, theft, donor stewardship, or other financial issues.  

Regulatory/Research and Compliance  

Concerns related to grants or research, falsification of documents or reports, athletics, and violations of local, state, federal or other regulations. 

Risk and Safety 

Unsafe working conditions, vandalism, environmental issues, theft of university property, personal use of university materials or assets and other risk and safety issues.

Information Technology 

Software piracy or copyright infringements, data privacy or integrity, inappropriate use of technology.

Medical  

Medical issues or concerns relating to University medical research, operations or programs.

Is the reporting party's name confidential?

 

Reports made in the Lighthouse System and associated investigatory records will be kept confidential to the extent possible under applicable state and federal law. Be advised that revealing your identity to Lighthouse may result in disclosure of your identity in response to a public records request under Florida’s public records law [Chapter 119, Florida Statutes].
