Health Immunizations

All beginning students must submit Health Immunization records to the UNF Office of Medical Compliance. All records must be submitted and cleared prior to attending orientation.
 

Please make sure your name and student "N" number are printed clearly on all forms. Students can submit health records by mail, fax or on-campus:

 
Once you have submitted your records please confirm with the Office of Medical Compliance that your records have been received and cleared. Please do not assume your health records have been received by UNF via your high school or college transcripts; they should be submitted separately.

Students who fail to submit and clear their health immunization records prior to orientation have a hold placed on their registration and will not be able to register for classes. Please note, the orientation sessions are very busy and you will not have time to clear these records while you are here for orientation.
