IMPORTANT! The APC forms to submit courses and programs for review have also been updated. These forms are now in .pdf format including help text for each field. |
|
Please do the following immediately to ensure your future APC submissions are in the correct format: |
|
1. |
Delete all previous versions of the blank APC forms you have saved to your workstation or storage media (Microsoft Word files). |
2. |
Download the *newest versions* of the blank APC forms to use for all future requests (Adobe .pdf files). |
3. |
Verify you have Adobe Acrobat 5.0 or higher on your workstation. You will need Acrobat (not Reader) to be able to edit and save your APC forms. If you do not see Acrobat 5.0 or higher on your programs list, you will need to contact the ITS Support Center to request this installation. |
4. |
Review the ‘ Online APC Forms & Resources’ website including the Supplemental Instructions for these forms. |
|
|
NOTE: You do NOT need to resubmit forms in the new format if they are currently in the review process at a departmental level or higher as of the date of this FA agenda. |
