If you wish to use FormProc to develop a survey or form, please read the following announcement that was published in Campus Update on October 3, 2006.

Information Technology Services (ITS) and the Center for Instruction & Research Technology (CIRT) have launched an online survey application called WebSurveyor. WebSurveyor is used to create web-based surveys and forms as well as to collect results to view and analyze. WebSurveyor is replacing FormProc, the tool used previously to create surveys and forms.

Getting Started:

• Faculty request an account by contacting CIRT at ext. 3927
• Staff request an account by contacting the ITS Support Center at ext. 4357 (HELP)
  or submitting a request at http://help.unf.edu
