4.1 Making the Submittal Form

Before setting up FormProc to work with your page, you will need to create the HTML form. Webcom has an excellent forms tutorial available. Many HTML editors also have forms-creation capabilities.

4.2 Configuration Form

Fill out the following form and press the "Submit Request" button in order to setup a form for use with FormProc. You will probably want to do this for each form you have, unless every form will have the same exact setup. If all of the forms have the same setup, you can just use the same configuration for all of them. You should expect that it will take at least three business days for a request to be filled.

4.3 Contacting the Administrator or Author
