Vision Care Open Enrollment
The Office of Human Resources is holding a special open enrollment period for vision coverage. As mentioned during the state-sponsored open enrollment period, the state will not offer a pre-tax supplemental vision plan for 2008. UNF has arranged to have this plan continued as a post-tax benefit. The premium, benefit and provider network will remain the same. The CompBenefits vision plan provides an extensive network of optometrists and ophthalmologists and provides you with a benefit option that covers all routine eye care, including eye exams plus eyeglasses (lenses and frame) or contacts.
Current participants who wish to continue their coverage must complete a new enrollment form and return it to the Office of Human Resources by November 29, 2007 to avoid an interruption in coverage. Please call extension 2903 with any questions.
