Nutrition is, as described by Merriam-Webster, “the act or process of nourishing or being nourished; specifically: the sum of the processes by which an animal or plant takes in and utilizes food substances."

For us to be properly nourished, our body needs many different types of nutrients such as; Proteins, Carbohydrates, Fats, Vitamins, Minerals, and Water. In each colored tab above is a brief description of functions and sources of each nutrient.

 

Nutrition Assessment


The Nutrition assessment is a personal 3-day food diary. The diary will determine the client’s daily caloric intake and break down his/her food consumption into numerous categories to determine daily intakes, such as vitamin, mineral, protein and fat intakes. In addition, the report will include nutritional values of the foods and a “recommendation” list based on your nutritional goals.

To complete a 3 day food diary for a nutritional assessment, please download and complete the following form and return to the Paul Shirley Assessment Center.
