On all financial transactions and documents, please include the Banner Index , as well as the Fund, Org, Program and Account codes that will be charged with the transaction amount.

If you are unsure of the correct Index that relates to the Fund & Org combination that is to be used in the transaction, the Banner Account Index Query (FTMACCI) will assist you in determining the index to be used.
