Upon the successful defense of the Comprehensive Examination and advancement to candidacy, the student shall declare a Dissertation Committee in accordance with the provisions stated in 8.2.2 using the links stated below for the appropriate forms. Selection of the Dissertation Committee is the joint responsibility of the student and faculty research mentor. The Dissertation Committee is composed of a minimum of four members of the UNF graduate faculty.
Students must consult the website of the UNF Graduate School for the appropriate dissertation committee membership forms required:
1. Dissertation Committee Membership Form
2. Request for Non-Graduate Faculty Dissertation Committee Member Form (inside UNF) - (If applicable) OR
3. Request for External Dissertation Committee Member Form (outside UNF) - (If Applicable)
Note: The doctoral student will submit either Forms 1 and 2 or Forms 1 and 3.
For both the proposal and the final dissertation the student shall use the approved template and program guidelines for formatting, language use, presentation and American Psychological Association citation style.

When the signatures in 8.5.4 have been obtained, the department will notify the candidate, who is responsible for retrieving the document from the department office, preparing the required documents, and submitting all to the Graduate School. The verified version of the dissertation must be uploaded to the Carpenter Library Digital Commons not later than two weeks before graduation date of the term in which the student is graduating -, and filing an application for ProQuest/UMI publication with the Graduate School. The student is required to submit the following items to the Graduate School two weeks prior to graduation.
Upon approval by the Dean of the Graduate School, the Graduate School will submit all necessary documentation for deposit of the dissertation in the UNF Library.

The award of degree is not complete until the Graduate Dean has approved the document, so that the Department Chair may certify graduation. There is a published deadline each semester for securing approval of the Graduate Dean. Failure to meet this deadline may result in a change in the official date of the award of the degree. The deadlines are established in each academic year and generally fall in the second week immediately following commencement. Students should review both sets of requirements carefully.
