EPI Payment Information
PAYMENT OPTION 1: Single Payment
- Pay $1500.00 by August 30, 2008.
PAYMENT OPTION 2: Scheduled
Payment
- Payment 1: $500.00 by August 30,
2008.
- Payment 2: $500.00 by October 5,
2008.
- Payment 3: $500.00 by November
5, 2008.
Fee
Payment Form
Program enrollment will be terminated
if the total program fee of $1500.00 is
not paid by 11/5/08.
Tuition Refund Policy
How to file for EPI withdrawal with
full or partial tuition refund:
1. Refunds are processed through the EPI
office and not the department where you
sent your program payment. Direct all
questions to Dr. Betty Bennett at
904-620-1823.
2. Email Dr. Betty Bennett at
bbennett@unf.edu to explain your
reason for withdrawing from the EPI and
request a tuition refund.
3. Dr. Bennett will send you the
decision regarding your request for
refund according to the written policy
in this document for each option.
4. $50.00 processing fee:
Non-refundable.
EPI Refund Policy for Option 1:

100%
tuition refund allowable if your
emailed request is received in
the EPI office no later than
August 25, 2008.
-
50%
tuition refund allowable if your
emailed request is received in
the EPI office from August 26
through September 26, 2008.
-
Extreme and extenuating
circumstances, verified through
submitted documentation, may
result in a 100% allowable
refund for EPI members who have
actively participated in the EPI
up to the point of emergency and
who email a request for refund.
Extreme and extenuating
circumstances include:

A
participant’s involuntary
call to active military
duty.
-
A
death in the participant’s
immediate family.

Incapacitating illness of
such duration of severity
that completion of the
program is impossible. The
condition must be confirmed
in writing by a physician.

Other exceptional
circumstances beyond the
participant’s control that
prevent continued
participation in the EPI.
Circumstance(s) must be
described in writing. No
refund will be made for
teachers whose positions are
terminated or who resign for
any reason while enrolled in
the program.

No
refund will be made to
individuals who fail to meet
criteria listed in items 1, 2,
or 3 in this section. An
individual who neither applies
for a refund nor shows up for
class forfeits all program fees.
EPI Refund Policy for Option
2:
- Payment 1 may be refunded as
follows:
- 100% tuition refund
allowable if your written
request for refund is received
in the EPI office no later than
August 25, 2008.
- 50% tuition refund allowable
if your written request for
refund is received in the EPI
office from August 26 through
September 26, 2008.
- Payments 2 and 3 are
nonrefundable.
- Extreme and extenuating
circumstances, verified through
submitted documentation, may result
in a 100% allowable refund for EPI
