All steps are required to submit a complete application.
If students submit complete applications prior to arrival
in Jacksonville, UNF will be able to register students
for courses at that time. If students choose not to submit
all required materials, they are choosing to have late
course registration. There is no guarantee on course
availability if applications are submitted incomplete
or late.
The
application deadline is October
1 for Spring Term and May
1 for Fall Term. |

There is a different form for students attending UNF for
one semester only and students attending UNF for two semesters. |

• |
All international students are
required to have international insurance with adequate
coverage. Students will be automatically enrolled and billed
for UNF insurance when they register for classes. If they
have existing insurance coverage, meeting UNF requirements,
these students should ask their insurance company to complete
the appropriate section of this form and return it prior
to arrival in Jacksonville. If this evaluation form is
not received by August 1 (for fall) or November 1 (for spring), students
will be automatically enrolled in the UNF insurance.
Note: A copy of the international insurance contract will not be accepted as an alternative to the evaluation form. |

Once arriving in Jacksonville,
students can pay for their International Insurance at UNF’s
cashiers office with cash or check. Students can pay on myWings with
a credit card. Please pick up a temporary insurance card
at the Office
of Medical Compliance. This office can also explain
the policy and claims procedure. |

provided by the Medical Compliance Office. |

This Immunization Form information
is required for ALL students on UNF’s campus. Students
will not be permitted to register for courses until they
have provided official documentation for these immunizations. |

Hepatitis
B: Students may provide proof of this immunization
or sign a waiver. Please read all information provided
by the Medical Compliance Office about Hepatitis B. |

Meningitis:
Students may provide proof of this immunization
or sign a waiver. Please read all information provided
by the Medical Compliance Office about Meningitis. |

All students born after 1956
must submit documented proof of immunity to Measles and
Rubella. There is a DETAILED EXPLAINATION of these requirements
on page 2 of the Immunization form. |

the Exchange Officer at their home institution. |

brief descriptions listing of course content and prerequisites. |

Housing |
|
|
|
• |
is recommended that exchange
students live on UNF’s campus. Our Housing
Department has a comprehensive web site which describes
housing options, accommodations, prices, locations and
general information. |
• |
If students would like to make
alternative arrangements to live off-campus, they must
note this at the time they submit their application. Students
will be asked to submit the name and location of their
alternative housing arrangements, too. |

Please submit a copy of your transcript (or another document that lists course titles and grades earned) from all universities you have attended. |
