Information for UNF Camp Directors

The University of North Florida's new regulation, "Camps and Similar Other Programs Involving Minors," requires VP approval for all existing and new camps at the University. This site serves to provide information to faculty, staff, camp directors and camp sponsors including instructions, forms and necessary links to complete the camps process to camp directors.

Camp Application   The camp application must be completed and submitted to the Office of Environmental Health & Safety within 6 weeks of the start of a camp on campus. 
     
Outside Activity Report  

You must submit this form if you are a 9 or 12 month faculty member, full-time A&P or USPS staff member and if:  you propose to engage in any compensated or professional outside activity as described on the form and/or you propose to engage in any outside activity, compensated or uncompensated, for which you should reasonably conclude:  may create or reasonably appear to create a conflict of interest, OR may otherwise interfere or reasonably appear to interfere with the full performance of your professional responsibilities or other institutional obligations.

     
Camp Employee Background Checks   This list must be included in the camp application package and submitted at least 2 weeks prior to the start of camp.
     
Camp Participant List   A preliminary list of all camp participants for each session must be included in the camp application package and submitted 2 weeks prior to the start of camp.
     
Camp Insurance Requirements and Information   An overview of the camp insurance program requirements and information regarding the purchase of camp insurance. All insurance forms must be submitted to EH&S at least 2 weeks prior to the start of camp.
     
Affidavit of Good Moral Character   Required for all adult participants in camps.

    Liability Waiver and Release Forms
     
Third Party Release   Non-UNF entity that operates a camp on UNF's campus.  This release is to be signed by the non-UNF entity, not by camp participants.
     
Volunteer Acknowledgement   Adult Volunteers - For situations in which an adult volunteer (unpaid) will be asisting with the camp.  The acknowledgement is signed by the adult volunteer.
     
General Activity Release   Adult Campers - For situations in which an individual over the age of eighteen (18) will be participating in a camp sponsored by UNF.  This release is to be signed by an adult camper.
   

Minor Event Release   Minor Campers - For situations in which an individual under the age of eighteen (18) will be participating in a camp sponsored by UNF.  The release is to be signed by the parent or guardian of the minor camper.
