Name

Criteria

Lynne M. Sneed Endowed Scholarship

Meet admissions requirements and be accepted to UNF, minimum UNF GPA of 2.5, enroll in BCH as a full or part-time student, demonstrate financial need as defined by the University’s Office of Financial Aid, and complete the Free Application for Federal Student Aid (FAFSA).

Paul and Virginia Shields

Meet admissions requirements and be accepted to UNF, meet admission standards for a major at the junior level, demonstrate financial need and complete the Free Application for Federal Student Aid (FAFSA).

Brooks Health Scholarship

Meet admissions requirements and be accepted to UNF, minimum UNF GPA of 3.0, complete the Free Application for Federal Student Aid (FAFSA).  Preference will be given to students whose interests are in physical therapy, nursing, athletic training and rehab counseling. 

Glenmoor/Vicar’s Landing Scholarship

Enroll as a full-time or part-time junior accepted into BCH, express an interest in geriatric management, earn a minimum undergrad GPA of 3.0

Baptist Medical Center –Nassau

Meet admission requirements and be accepted to UNF,  reside in Nassau County, enroll as a full-time or part-time junior or senior student in BCH and earn a minimum undergraduate UNF GPA of 3.0

Memorial Hospital Auxiliary Endowed Scholarship

Complete the free application for Federal Student Aid (FAFSA), enroll as a full-time junior or senior or full-time grad student in BCH in a minimum of 12 credits for undergrad or 9 credits for grad, maintain a GPA of 2.5 undergrad or 3.0 grad

Susan Joy Smith Memorial Endowed Scholarship

Reside in Duval County, enroll as a junior in a program in BCH, demonstrate financial need, complete the free application for Federal Student Aid (FAFSA), enroll full time (12 credits) each semester and earn a minimum of 24 credits by the end of each academic year, maintain a GPA of at least 2.5

Exercise Science Military Service Recognition Scholarship

Meet UNF admission requirements and be accepted to the BCH Exercise Sciences program, preference will be given to a student who has served in the military or is the family of someone who serves (or has served) in the military, has a cumulative UNF GAP of at least 3.0, completes the Free Application for Federal Student Aid (FAFSA), and demonstrates financial need as defines by the Office of Financial Aid.

Graduate Assistantship and Partial-Waiver Packages

Graduate Assistantship/Partial tuition waiver packages, partial tuition waiver only packages

All financial aid discussed below is subject to change depending on Brooks College of Health allocations
For the coming academic year, the Brooks College of Health anticipates awarding several graduate assistantship/partial tuition waiver packages and several partial tuition waiver only packages (without graduate assistantship). All of these packages are for students new to BCH graduate programs, and will be granted for a maximum two year period for those students maintaining eligibility, and satisfactory performance in the assistantships. Occasionally students relinquish their packages prior to the end of the two year award. In these cases the remainder of the award will be given to another student, however the award will end at the time the original award was scheduled to end.
NOTE: All fee waivers are partial, and do not cover the total cost of fees. All fee waiver packages are subject to funding.

Eligibility Requirements:
Eligibility requirements for all partial waiver/graduate assistantship and partial waiver only packages are as follows:
The applicant must:
   1. be a new, fully admitted student to a graduate program in the BCH;
   2. be a full-time graduate student enrolled in a minimum of 9 credit hours per semester;
   3. have a minimum admission GPA of 3.0 on a 4.0 scale.
Requirements for continued eligibility requirements for the remainder of the award period: The award recipient must:
   1. maintain a GPA of 3.0 on a 4.0 scale while enrolled in the graduate program;
   2. maintain full-time enrollment in the BCH graduate program (minimum of 9 credit hours per semester);
   3. maintain satisfactory performance as a graduate assistant (if applicable).

Application process: Applications are available on-line:
1. Prospective students submit two applications: one application goes directly to the Chair of the Department where the program applicant is interested in is located (for nursing applicants, submit to the Director of the School of Nursing). Submit the application by the individual program’s admission deadline (or July 15th, whichever is the earliest) (see program specific information located elsewhere on the BCH website).
2. The prospective student then submits a duplicate application to the Associate Dean’s office, to the attention of Judith Kraft.
3. Both applications should be submitted via e-mail as an attached MSWord file that is labeled: BCH Waiver App. (insert student’s last name); applications may also be submitted via regular mail.
Applications should be submitted separately to Ms. Kraft and the appropriate Department Chair/School Director.
4. The application deadline is the admission deadline for the program the student is interested in, or July 15, whichever is the earliest.
5. Applications received by the Associate Dean’s office will be acknowledged via e-mail or regular mail.

Graduate Assistant Duties:
Graduate assistants are assigned to faculty and work as a research assistant or a teaching assistant, depending on the needs of the faculty person. Graduate assistants are expected to work approximately 10 hours per week for a maximum of 150 hours total for each semester. The work is compensated at the rate of $10.00 per hour or a total of $1,500 over the course of the semester.
