All beginning students must submit Health Immunization records to the UNF Office of Medical Compliance. All records must be submitted and cleared PRIOR to attending orientation.
Student can submit health records by mail, fax or on-campus.

IMPORTANT: Please make sure your name and student “N” number are printed clearly on all forms.
Once you have submitted your records please confirm with the Office of Medical Compliance that your records have been received and cleared. Please do not assume that your health records came over on your high school transcripts; they should be submitted separately.
Students who fail to clear their health immunization records PRIOR to orientation will have a hold placed on their registration and will not be able to register for classes on the day of orientation. Please note: the orientation day is VERY busy and you will not have time to clear these records the day of orientation.
