For International Scholars

The US Department of State requires all international scholars maintain health insurance coverage during their stay. Please read the requirements carefully. Insurance must meet the minimum Federal requirements.

J-1 Insurance Notification Form 

Scholars have the option to purchase insurance that meets the minimum medical requirements from the UNF Medical Compliance Office, (904) 620-2175. Scholars that wish to use an alternate medical insurance policy must confirm minimum requirements using the form below:
