I. OBJECTIVE & PURPOSE
Create a policy to require a Statement of Good Health for all International students with Visa status of F1 & 2; J1 &2 prior to registering for on campus classes. This proposal is based upon prior BOR policy and existing SUS practice.
II. STATEMENT OF POLICY
All International students must provide the University with a Statement of Good Health prior to enrolling in classes at the University of North Florida. This will help to insure the health of International students as well as reduce the risk of infectious disease to those who may come in contact with them.
The following information will be required for approval:
- Medical provider’s Statement of Good Health.
- Medical provider’s signature.
- Medical provider’s official stamp or address stationary.
*This is a one-time requirement and current International students will be grandfathered in.
