ADA Compliance Office Procedures and Processes

Accommodation Requests for Faculty, Staff and Applicants:

To request an accommodation for faculty, staff and applicants, please contact the ADA Compliance Office Staff at (904) 620-2870.

Unless to do so would cause an undue hardship, Title I of the Americans with Disabilities Act (ADA) Amendments Act of 2008 provides fundamental statutory requirements for an employer to provide a reasonable accommodation to qualified individuals with disabilities. An ADA accommodation provides employees who are "otherwise qualified" i.e., the employee qualifies for the job except that because of disability s/he needs a reasonable accommodation to perform the position's essential functions. Typically, an accommodation is any change, modification or adjustment in the work environment made to enable an individual with a disability to enjoy equal employment opportunities.

 

When an employee desires an ADA accommodation, the employee should contact the ADA Compliance Office staff at telephone number (904) 620-2870 or by visiting the office located at Building 6, Room 1314. The employee will be provided a package with the forms to be completed by the employee and their medical provider. Once the forms are completed the employee and the medical provider will send them to the Director of ADA Compliance. The medical information will be kept in the ADA Compliance office and treated as confidential information. The employee will meet with the Director, ADA Compliance. The ADA Director will begin the interactive process with the employee and supervisor(s). The Director of ADA will determine if the employee is a qualified individual with a disability and determine the reasonable accommodations. 

 

Similarly, a reasonable accommodation enables an applicant with a disability to have an equal opportunity to participate in the application process and to be considered for a job. Finally, a reasonable accommodation allows an employee with a disability an opportunity to enjoy the benefits and privileges of employment that employees without disabilities enjoy.

 

 

Visitor Accommodations:

Visitors requesting an accommodation should contact the department sponsoring the function to be directed to the proper department or you may call UNF's main telephone number at (904) 620-1000. The ADA Compliance Office provides information and assistance on ADA issues to the UNF faculty and staff and may be reached at (904) 620-2870. The Disability Resource Center provides information and assistance to UNF students and may be contacted at (904) 620-2769. Disability Parking:

The University provides parking spaces for drivers who have disabilities. If you display both a valid parking permit (Daily, Consecutive Multi-day or Vendor) for that lot or garage and either a state-issued disability parking placard or a UNF-issued temporary mobility impairment placard, you may park in a disability parking space. Additionally, the University provides shuttle buses that are equipped with wheelchair ramps and are ADA compliant. Course Substitution Requests:

The Disability Resource Center (DRC) should be contacted for a Course Substitution Request for Foreign Language or Math. The Director, ADA Compliance Office is involved in the processing; however, the student making the request should contact the DRC for forms, decision results and a copy of the signed request. Appeals Process for DSAC (Disabilities: Student Admission Committee) Decisions

If a student applying for admission to UNF voluntarily checks the box to self identify as a person with a disability, the Disabilities: Student Admission Committee (DSAC) will review and make the decision about the student’s admission. If the student who self identifies as a person with a disability is denied admission and believes the reason for denial was due to discrimination, lack of sufficient information, or other significant issues, the student may submit an appeal of the admission decision by the DSAC.

 

To appeal the DSAC decision, the student must send a letter stating the reason(s) for the appeal and provide information supporting the reason for the appeal. The appellant’s letter must be sent to the Director, ADA Compliance Office within 10 working days of receipt of the decision from University’s admissions office. Once the written request is received, the Director will review all the information provided and the information considered by the DSAC. The appellant will receive a response within 15 working days from the date the letter was submitted to the ADA Compliance Office.
