Admissions decision appeal form

For an appeal to have merit, there must be new academic or personal information, as well as information pertaining to extenuating circumstances that were not present in the initial application. Each appeal receives a holistic review by faculty and additional University staff members. 

 

Your appeal must contain:

 

  • A personal statement
  • New information to support the appeal since the initial admission decision

Only complete appeals will be reviewed by the committee. All appeals are considered based on their merits and all decisions of the committee are final.

Personal Statement

Please type a statement addressing why you believe you should be reconsidered for admission, including an explanation of any circumstances affecting your previous academic performance. Your statement should not exceed more than 500 words.

If yes, please submit documentation verifying your disability. The documentation must be supplied by a qualified professional who is licensed for certified to diagnose the disability in question. If you had an Individualized Learning Plan in high school, this is valuable resource of information but it cannot be used as documentation of the disability.
